Form R: Registering for Postgraduate Specialty Training

Please note that anyone registering for General Practice Specialty Training will be contacted directly by the GP Administration Team, therefore do not need to download Form R from the website.

Every trainee within the Deanery, with the exception of existing SpRs who are training using the old curriculum, is required to complete ‘Form R: Registering for Postgraduate Specialty Training’, annually. Without a completed Form R the Deanery cannot register you on the Postgraduate Dean’s Database or issue/continue you with your National Training Number (NTN issued to run-through trainees only). Doctors appointed to a Fixed Term Specialty Training Appointment (FTSTA) will not receive an NTN but Deaneries will keep a record of trainees appointed to FTSTAs and their achievement of competences.

Once the Deanery has received a trainee’s Form R, it will be forwarded to the relevant College to complete the registration process. Due to the large volume of Form Rs received, the Deanery will not acknowledge receipt of the Form R, nor will they send you a copy of your Form R with the Postgraduate Dean’s signature. New run-through trainees will however receive an email confirming their National Training Number and their PMETB Approval Code.

Please ensure you read the guidance to assist you to complete Form R.

Please download Form R and the Conditions of Joining and return both documents completed and signed, together with a pasport sized colour photograph (signed and dated on the back), to the following address:
