This is an example only of the Agreement which must be signed by all parties and returned to the Deanery BEFOREany training should commence.

Educational Agreement between Postgraduate Dental Dean, Trainer and Competency Applicant

Competency Applicant Address (“the Applicant”)

The purpose of this agreement is to set out the responsibilities of the Trainer and the Applicant

As the Trainer named above I agree to carry out the duties listed below for the period of competency training recommended by the Postgraduate Dental Dean with the agreement of the PCT.

To maintain and provide a dental practice which meets Clinical Governance requirements of PCT and the standards required of Vocational Training

The Applicant should work only at the practice which has been inspected and approved by the Postgraduate Dental Dean or his representative unless prior agreement of the Deanery has been obtained.

To inform the PCT and Postgraduate Dental Dean as soon as possible of any changes which may occur which are likely to affect the performance of this contract.

To work in the same premises, in close proximity and allowing access to the Applicant a period of not less than 3 days or 6 sessions of not less than 3 hours duration.

To provide the Applicant with adequate administrative support and a full time suitably experienced Dental Nurse.

To conduct an initial PDP interview with the assistance of the Deanery PDP tutor/ VT adviser to identify strengths and weaknesses and set up a PDP with these factors in mind.

To be prepared to monitor progress of the Applicant and be available to give guidance in both clinical and administrative matters when requested and when necessary.

To prepare and conduct regular weekly tutorials within normal practice hours of at least 1 hour free from outside disturbance and document them fully in the PDP.

To allow and require the Applicant to attend postgraduate courses identified by you and the Postgraduate Dental Dean/PDP Tutor as needed to support the competencies required for the Competency Application to proceed.

To monitor the Applicant’s workload throughout the training period and ensure that it is sufficient to allow experience of a wide range of treatment, but not too onerous that it may interfere with the training process.

To ensure that the PDP is completed by the Applicant and the processes involved in assessment are documented and kept up to date.

To allow access to the dental practice and all documentation by the deanery staff as required

To sign and adhere to an appropriate contract of employment for the Applicant drawn up with the advice of the BDA

Any breach of the obligations listed above may be reported to the PCT who will consider the future of this training process.
